Purpose of This Form

The Saratoga Regional YMCA is an organization grounded in strong values of responsibility and integrity. We are committed to maintaining an environment where open and honest communication is encouraged. You should always feel comfortable approaching your supervisor or management with concerns about potential violations of policies or standards.

This whistleblower complaint form is specifically intended for reporting suspected illegal, unethical, or fraudulent activities, including but not limited to:

  • Fraud, theft, or embezzlement
  • Financial misstatements or accounting irregularities
  • Conflicts of interest
  • Violations of laws or regulations
  • Misuse of donor funds or grant funding
  • Ethical violations by leadership
  • Retaliation against whistleblowers
  • Significant breaches of organizational policies
  • Safety violations or criminal activity

What This Form Should Not Be Used For

Concerns related to routine human resources matters, workplace disagreements, performance issues, or general employment grievances should be directed to the HR Department through standard HR complaint procedures. These issues are not handled through the whistleblower process.

Examples include:

  • Routine HR complaints (e.g., performance disputes, personality conflicts)
  • Grievances covered by your existing Employee Complaint or Grievance Policy
  • Harassment or discrimination complaints (if you have a separate reporting path)
  • Day‑to‑day workplace concerns

We guarantee that your concerns will be reviewed and that no retaliatory action will be taken against anyone who reports or inquires in good faith about potential policy violations or who seeks guidance on addressing suspected misconduct.
